UPSMFAC Combined Paramedical Selection Process (CPSP) Online Form 2022

▶ Educational Qualification :-

इण्टरमीडिएट विज्ञान वर्ग ग्रुप परीक्षा या उसके समकक्ष परीक्षा जो कि केन्द्र अथवा उ०प्र० शासन से मान्यता प्राप्त हो (ये सभी उ0प्र0 माध्यमिक शिक्षा परिषद बोर्ड के समकक्षता प्राप्त होना चाहिये) एवं बायोलाजी ग्रुप अथवा गणित ग्रुप से भौतिक विज्ञान एवं रसायन विज्ञान सहित विषयों से उत्तीर्ण हो। समकक्षता न होने की दशा में अभ्यर्थी का प्रवेश / चयन होने के पश्चात् भी प्रशिक्षण से विरत किया जा सकता है। 40% कम अंक के अभ्यर्थी आवेदन नहीं कर सकते।

2. Click on button
3. Read the instructions carefully and check the “I have read the prospectus and the instructions carefully and understood them” check-box. Click on Continue button.
4. Fill the complete Applicant’s Information like First Name, Middle Name, Last Name, Mobile Number, E-mail etc. as shown within the Applicant Registration Panel.
5. Fill the Academic Qualification Details as required and explained below: High School (Name of Board, Name of College, Year of Passing, Roll No., Percentage) And Intermediate (Name of Board, Name of College, Year of Passing, Roll No., Percentage)
6. Fill the Marks Entry deatils of Intermediate Subjects : Physics Marks Obtained • Chemistry Marks Obtained • Maths Marks Obtained OR • Biology Marks Obtained • Subject – 1 (Name of Subject-1) • Subject – 1 Marks Obtained • Subject – 2 (Name of Subject-2) • Subject – 2 Marks Obtained
7. Check on the Declaration and SAVE the details.
8. The applicant will get the UPSMFAC ID and Password for the futher use and requested to save it for further use.
9. An E-mail will be sent to your provided E-mail consisting of UPSMFAC ID and Password.
10. Login with the UPSMFAC ID and Password
11. Upload the followings: Photograph, Signature, Highschool Marksheet, Intermediate Marksheet
12. Click on Payment to go to make an Online Payment. Don’t click on Back button while making payment. There are two online payment modes 1. Credit Card 2.Debit Card 3. Net Banking (Follow the instruction shown on the screen as per your choice)
13. Click on Print Form to get the printout of the completely filled application form after making the payment.
